ABOUT

High Plains Community Health Center (HPCHC) is a Federally Qualified Health Center located in Southeastern Colorado that is dedicated to the prevention of disease and access to improved healthcare through the promotion of integrated primary care, dental and behavioral health services. Serving nearly 10,000 patients from underserved populations across nine counties in two states, HPCHC offers imaging, care coordination, lab, and other support services to ensure that patients in these rural communities receive the most comprehensive care in the region.

OUR MISSION

HPCHC will be the provider of choice for quality, accessible primary health care services in Southeast Colorado.

Located in the town of Lamar in Prowers County, Colorado, HPCHC opened in 1995 as the result of a volunteer effort to address the lack of health care in the area. Prowers County, itself, is largely rural, with approximately eight people per square mile. Prior to 1995, the county’s population of 14,000 was served by just two primary care physicians. Because of the high demand, HPCHC expanded rapidly to include dental and behavioral health and an onsite dispensary as well as primary care services.

HPCHC’s current staffing includes just under 100 full- and part-time positions. Primary care, dental, behavioral health and administrative services are all housed on our main campus. Additionally, HPCHC staffs a clinic in Holly, Colorado, located 5 miles west of the Kansas border.

POSITION SUMMARY

The Chief Medical Officer (CMO) provides administrative and clinical leadership and oversight to the Medical, Dental, and Behavioral Health Departments to ensure patients receive high quality care.  The CMO provides expertise and consultation to ensure regulatory and program requirements are met.  Oversight of the integration of Medical, Dental, and Behavioral Health Departments to ensure evidence-based and patient centered practices are provided to all patients.

PRODUCTIVITY AND EXPECTATIONS:

The CMO position is an essential component to the success of HPCHC. This position is expected to prioritize time and responsibilities in the following manner:

20%: Executive Leadership

  • As a member of the Executive Leadership Team, the CMO will participate in strategic planning and development of objectives to meet short- and long-term goals.
  • Leads committees as assigned, participates in the Board of Directors Meeting, Board Credentialing and Privileging Meeting, Compliance Committee, Risk Management Committee, and Quality Committee.
  • Reviews survey reports of any authorized agency and assists executive administration with ensuring that HPCHC meets the standards of all accrediting and certifying bodies and licensure agencies.
  • Oversees the recruitment, selection, training, supervision, and evaluation of medical staff. This includes the implementation and oversight of an orientation program for Clinic’s onboarding of providers.
  • Ensures that policies and procedures of HPCHC are followed with the Chief Compliance Officer.

40%: Patient Care

  • Manages acute and chronic illness, develops a plan of care, coordinates care and determines the need for case review.
  • Makes appropriate referrals to both internal disciplines of the clinic and/or external agencies for additional services required.
  • Provides clinic consultation to providers as needed.
  • Assists with planning to evaluate provider performance levels, quality of service, productivity, and future needs.
  • Knowledge of local agencies to ensure collaboration of patient care.
  • Provides leadership and assists in efforts to improve the efficiency and effectiveness of the clinics.
  • May establish a mutually structured prescribing mentorship of 750 hours with a Nurse Practitioner during their provisional prescriptive authority period.
  • May establish a collaborative practice agreement with an APP.
  • Involved in the development of workflows, offering ideas, opinions, and concerns, and demonstrating “ownership”.
  • Accountable for implementation and sustainability of processes and workflows for the care team.
  • Indicates awareness of staffing, scheduling process, scope of practice, policies, and procedures, and is focused on patients-centered care and team cohesiveness.

20%: Medical Operations

  • Supervises and guides the Director of Clinical Operations to ensure they have the tools needed to directly oversee the Medical Department and its clinicians.
  • Works with the Medical Department to ensure productivity and QI/QA standards are met and compliance is achieved through optimal department functioning.

10%: Behavioral Operations

  • Supervises and guides the Director of Behavioral Health to ensure they have the tools needed to directly oversee the Behavioral Health Department and its clinicians.
  • Works with the Behavioral Health Department to ensure productivity and QI/QA standards are met and compliance is achieved through optimal department functioning.

5%: Dental Operations

  • Supervises and guides the Director of Dental Services to ensure they have the tools needed to directly oversee the Dental Department and its clinicians.
  • Works with the Dental Department to ensure productivity and QI/QA standards are met and compliance is achieved through optimal department functioning.

5%: Grants

  • Assists with the application process and required reporting for grants that would complement the Clinical Departments.
  • Complete required administrative duties for grants.

QUALIFICATIONS

  • Current MD or DO license in the state of Colorado.
  • Board Eligible or Board Certified.
  • Current Drug Enforcement Administration (DEA) license.
  • Current Basic Life Support (BLS) certification.
  • Ten years working in the health care field is required.
  • Five years experience in executive leadership is required.
  • Experience working with a diverse population and/or underserved populations preferred.
  • The position requires a working knowledge of modern office practices and This includes but is not limited to:
    • Proficiency in using the Internet, Microsoft Office programs and general computer
    • Skill in operating office equipment such as copier, fax machine, scanners,
  • Possess customer service skills necessary to provide a positive experience necessary when they speak to employees in person and on the phone.

  • Works both independently and as part of a team; establishes and maintains effective working relationships with health care team members and diverse patient populations.
  • Displays time management and organizational skills to consistently meet deadlines.
  • Performs other duties as required.
