Chief Medical Officer – Position Summary – The Chief Medical Officer (CMO) provides administrative and clinical leadership and oversight to the Medical, Dental, and Behavioral Health Departments to ensure patients receive high quality care. The CMO provides expertise and consultation to ensure regulatory and program requirements are met. Oversight of the integration of Medical, Dental, and Behavioral Health Departments to ensure evidence-based and patient centered practices are provided to all patients.
Behavioral Health Provider – Position Summary – The Behavioral Health Provider will provide behavioral health assessment, diagnosis, on-going therapy, and provide same day and crisis interventions to patients of High Plains Community Health Center (HPCHC). We practice an integrated, patient centered care model.
Dental Hygienist – Position Summary – The Dental Hygienist will provide dental hygiene treatment and preventative instruction in accordance with their administrative and clinical duties. They will be expected to communicate effectively to provide an exceptional experience for our patients.
Dentist – Position Summary – The Dentist is a position that will be responsible for oversight of Dental Hygienists, dental support staff, and provides treatment and prevention instruction in accordance with best practice in oral health care. This position will work both independently to provide an exceptional experience for our patients.
SafeCare Parent Support Provider – Position Summary – The SafeCare Parent Support Provider will work with a straightforward, evidence-based intervention program through the Kempe Center. SafeCare Colorado is voluntary, evidence based, home visiting program with specific protocols to work specifically with parents in served communities. This position will work primarily in the home setting with parents to meet these protocols by providing assessments, modeling behaviors, role playing, and provides feedback to parents in the SafeCare program.
PRN Housekeeping Specialist – Position Summary – Part-time as needed position. This position requires a high school degree or equivalency, certification or licensing complementary to training and experience. The Housekeeping Specialist will utilize infection control principles to maintain a clean and safe environment for patients, employees, and visitors.
