Position Summary

The Licensed Practical Nurse (LPN) is a position that will assist providers, patients, and staff before, during, and after patient appointments.  This position will work independently under the direction of their manager and provider team to provide an exceptional experience for our patients.

Productivity and Expectations:

The LPN position is an essential component to the success of High Plains Community Health Center (HPCHC). This position is expected to prioritize time and responsibilities in the following manner:

80%: Nurse-Directed Patient Care

  • Room patients in a warm and friendly manner, obtain and record vital signs.
  • Document a detailed Chief Complaint for the purpose of the patient’s visit.
  • Review medications, allergies, and past medical history (PMH).
  • Put in actions for every patient based on provider recommendations.
  • Perform medical procedures as directed by the provider and within scope of practice.
  • Update patient records and do all alerts as quickly as possible.
  • Identify, draw up, administer, and record appropriate immunizations and injectables.
  • Perform CLIA waived lab testing.
  • Perform and process blood draws.
  • Perform EKG testing.
  • Assist patients with all forms needed for the appointment.
  • Perform standing orders.
  • Accurately complete all required documentation in the electronic medical record.
  • Perform triage as needed. Determine priority based on assessments, critical thinking, observations and available information and schedule or triage patient appropriately for additional care. Refer to the emergency room for crisis intervention, if appropriate.
  • Initiate diagnostic and therapeutic interventions as indicated by standing protocols.
  • Utilize protocols to order refills and schedule necessary tests for prescribed medications. Schedule follow up appointments for medication management.
  • Identify appropriate tests and services needed for patients during annual wellness visit screenings and help patient schedule needed tests and services.
  • Educate patients regarding chronic disease self-management under the direction of a Registered Nurse or Medical Provider.
  • Understand and work to the standards of evidence-based care, review data, and receive feedback.
  • Understand the training protocols for support staff, participate in evaluation of core competencies, and be comfortable delegating tasks.
  • Involved in the development of workflows, offering ideas, opinions, and concerns, and demonstrating “ownership”.
  • Accountable for implementation and sustainability of processes and workflows for the care team.
  • Indicates awareness of staffing, scheduling process, scope of practice, policies and procedures, and is focused on patient-centered care and team cohesiveness.

10%: Chronic Care Management/Complex Care Management

  • Participate in daily ‘Huddle’ with provider and team.
  • Respond to phone messages before each work session ends.
  • Follow up with incoming testing results/referrals/diagnostic orders.
  • Coordinate care to promote timely access, increase preventative care utilization, reduce emergency room utilization and re-hospitalization. Care coordination to include scheduling appointments, monitoring treatment plans, and ensuring that patients receive appropriate services.
  • Assist in identification of “high-risk” patients (chronic illness and special health care needs).
  • Assess unmet patient health and social needs and develop care plan with the patient, family/caregiver(s) and providers (emergency plan, health management plan, medical summary and ongoing action plan).
  • Monitor adherence to care plans and patient progress, evaluate effectiveness and facilitate changes.
  • Connect patients to relevant HPCHC and community resources.
  • Provide chronic disease education to patients and families to increase ability for self-management and shared decision making.
  • Maintain accurate and up-to-date patient records, including documentation of patient care activities and tracking patient outcomes.

10%: General Duties

  • Maintain and stock exam rooms.
  • Clean and sterilize instruments following protocol.
  • Maintain and record vaccine refrigerator and freezer temperature logs.
  • Dispose of biohazardous waste following protocol.
  • Assist with training of new employees on an as needed basis per Supervisor. Assist with annual competencies for clinical staff.
  • Participates in Patient Centered Medical Home, Quality Assurance, Quality Improvement and Risk Management tasks per plans developed by HPCHC.
  • Performs quality control checks, follows infection control and Bloodborne pathogen standards and reporting requirements.
  • Completes online education modules as required.

Qualifications

  • Current and unrestricted Colorado LPN license.
  • Current Basic Life Support Certification (BLS) or within 90 days of hire is required.
  • Two years of experience working as a LPN in a medical setting with patients is preferred.
  • The position requires a working knowledge of modern office practices and procedures. This includes but is not limited to:
    • Proficiency in using the Internet, Microsoft Office programs and general computer skills.
    • Skill in operating office equipment such as copier, fax machine, scanners, etc.

  • Displays time management and organizational skills to consistently meet deadlines.
  • Performs other duties as required.

There will be opportunities for other duties as this position may evolve and policies and training may change. Reasonable accommodations will be reviewed, and modifications may be made according to ADA guidelines.  This position utilizes proper body mechanics, transfer/lifting techniques, and appropriate equipment to minimize fall risk to patients and/or injury to self.  This is an OSHA Category 1 position.
