Position Summary

The Nurse Practitioner is a position that will be responsible for the direction of clinical support staff and sees patients of all ages.  This position will work both independently and as a member of a patient centered medical home model to provide an exceptional experience for our patients.

 

Productivity and Expectations

The Nurse Practitioner is an essential component to the success of High Plains Community Health Center (HPCHC). This position is expected to prioritize time and responsibilities in the following manner:

 

80%: Patient Care

  • Provides services within accepted professional healthcare standards of care in accordance with safe and accepted patient care practices and current ethical standards and in accordance with a Nurse Practitioner’s best skill and judgement.
  • Practices healthcare and renders professional healthcare services at a level of expertise expected of a fully trained Nurse Practitioner in the execution of duties that include but are not limited to primary care and other duties reasonably appropriate to the practice of family healthcare.
  • Responsible to the Chief Medical Officer to work collaboratively on clinic issues that involve providers and be aware of changes that impact clinic staff.
  • Manages acute and chronic illness, develops a plan of care, coordinates care and determines the need for case review.
  • Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.
  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
  • Monitors the effectiveness of interventions.
  • Makes appropriate referrals to both internal disciplines of the clinic and/or external agencies for additional services required.
  • Knowledge of local agencies to ensure collaboration of patient care.
  • Provides leadership and assists in efforts to improve the efficiency and effectiveness of the clinics.
  • Involved in the development of workflows, offering ideas, opinions, and concerns, and demonstrating “ownership”.
  • Accountable for implementation and sustainability of processes and workflows for the care team.
  • Indicates awareness of staffing, scheduling process, scope of practice, policies, and procedures, and is focused on patients-centered care and team cohesiveness.

 

10%: Patient Documentation

  • Participate in daily “Huddle” with provider and team.
  • Respond to phone messages before each work session ends.
  • Follow up with incoming testing results/referrals/diagnostic orders.
  • Review referral documents and other patient related documents in EMR.
  • Prescription refills.
  • Coordinate care to promote timely access, increase preventative care utilization, reduce emergency room utilization and re-hospitalization.
  • Assist in identification of “high-risk” patients (chronic illness and special health care needs).
  • Assess unmet patient health and social needs and develop care plan with the patient, family/caregiver(s) and providers (emergency plan, health management plan, medical summary and ongoing action plan).
  • Monitor adherence to care plans and patient progress, evaluate effectiveness and facilitate changes.
  • Connect patients to relevant HPCHC and community resources.
  • Provide disease education to patients and families to increase ability for self-management and shared decision making.
  • Maintain accurate and up-to-date patient records, including documentation of patient care activities and tracking patient outcomes.

 

10%: General Duties

  • Provide mentorship to other clinical staff on an as needed basis.
  • Participates in the following meetings: Med Staff, Clinic Staff, Case Review (as needed), and other clinic related meetings as requested.
  • Responsible for Quality Improvement activities as directed by the Chief Medical Officer.
  • Responsible to the clinic providers to listen, communicate and advise in clinic provider related affairs.
  • May assist with reviewing and revising procedures of the clinics.
  • May participate in public functions on behalf of the clinics.
  • Completes online education modules as required.

 

Qualifications

  • Master’s degree from an accredited school of nursing.
  • Graduate of an accredited Nurse Practitioner Program.
  • Current RN license in the state of Colorado or a compact party state and board-certified by either ANCC or AANP.
  • Current Drug Enforcement Administration (DEA) license.
  • Current Basic Life Support (BLS) certification.
  • Valid Driver’s License.
  • The position requires a working knowledge of modern office practices and procedures. This includes but is not limited to:
    • Proficiency in using the Internet, Microsoft Office programs and general computer skills.
    • Skill in operating office equipment such as copier, fax machine, scanners, etc.

  • Works both independently and as part of a team; establishes and maintains effective working relationships with health care team members.
  • Displays time management and organizational skills to consistently meet deadlines.
  • Performs other duties as required.

 

There will be opportunities for other duties as this position may evolve and policies and training may change. Reasonable accommodations will be reviewed, and modifications may be made according to ADA guidelines.  This position utilizes proper body mechanics, transfer/lifting techniques, and appropriate equipment to minimize fall risk to patients and/or injury to self.  This is an OSHA Category 1 position.
